A high-powered microscope guides a systematic search for sperm within the testicles via a midline scrotal incision. Intraoperative analysis of the tissue is performed to identify sperm which can then be cryopreserved.

What To Expect During Surgical Sperm Retrieval Surgery

Before The Procedure

  1. Consultation: Your urologist will review your history, medications, and confirm your consent.
  2. Tests: Virology blood tests (HIV, hepatitis B, hepatitis C) are required within three months of the procedure to allow sperm freezing.
  3. Anaesthesia: Options include local anaesthesia (for PESA/TESA) or general/spinal anaesthesia (for TESE/microTESE), this can be discussed with your anaesthetist.

During The Procedure

  1. Duration: 30–180 minutes, depending on the method.
  2. Sperm storage: Retrieved sperm are frozen and stored by our affiliated fertility unit for future assisted reproductive techniques (ART) use.

After The Procedure

  1. Recovery: Expect mild swelling or discomfort for a few days. You’ll receive a scrotal support or can wear tight, supportive underwear.
  2. Pain relief: Paracetamol or ibuprofen can ease discomfort, and ice packs (not directly on the skin) may reduce swelling.
  3. Activity: Avoid sexual activity, heavy lifting, or strenuous activity for 1–2 weeks. Most patients resume work within 5–7 days.
  4. Follow-up: We’ll review your recovery and plan your next steps for fertility treatment.

Risks And Complications

Surgical Sperm Retrieval is a safe procedure, but potential risks include:

  1. Mild bruising and swelling: Common, affecting 1 in 2 to 1 in 10 patients.
  2. Possibility that no sperm is found: Varies by condition and technique
  3. Chronic testicular pain: Rare, between 1 in 50 and 1 in 250 patients.
  4. Testicular atrophy: Possible shrinkage requiring hormone therapy, between 1 in 50 and 1 in 250 patients.
  5. Infection or bleeding: Uncommon, between 1 in 50 and 1 in 250 patients, may need further treatment.
